Provider Resource Center User Registration

Step 1: Create your username and password. you will use this information each time you login to the PRC. Both your username and password must be at least five characters in length and can only contain letters, numbers, and/or an underscore.
Step 2: Enter you personal information so that we may identify you as an WQ Provider and activate your account.
Step 3: Click 'Submit'. An WorkWell representative will notify you by email when you account is activated.
